Sick notes

To request a sick note:

• Please be mindful this request may not be actioned in time for when you expect it to be actioned. Due to this request coming into our generic email address. If you have not had a reply after 7 days please contact the surgery through Systmconnect online or 01924 402073.
• Have you tried logging in to Systmconnect ( the banner is on our home page) Any request through this service will be actioned daily. 
• phone or visit the surgery from 8am to 6:30pm

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
